Heart to Heart, a heart failure community outreach program at The Valley Hospital, is specially designed for you and your caregivers to help manage your heart failure and improve your quality of life.
You will have access to nurse practitioners/clinicians who can help answer questions and clarify self-care management.
Need help managing your medication regimen? Bring us your medications and we will help you understand why you’re taking them and address any questions or concerns you may have.
Are you managing the care of a loved one with heart failure? This program offers support, tips for handling care, and a chance to meet others in similar situations.
Are you interested in hearing how others are maintaining their heart health? This program offers the opportunity to share thoughts, ideas, and experiences between those living with heart failure.
